Resumen
[ES]El objetivo global de esta Tesis Doctoral es profundizar en el estudio de varias cuestiones relativas al mecanismo de funcionamiento de estas proteínas transportadoras hepáticas con objeto de responder a varias preguntas de interés en áreas como la Fisiología, la Fisiopatología y la Farmacología. Esto ha llevado a definir tres objetivos parciales en el desarrollo de este trabajo:
- Investigar el mecanismo de transporte mediado por OATP1B1 y OATP1B3. Para ello se han utilizado diferentes modelos experimentales que han permitido esclarecer aspectos relacionados con su dependencia de cambios de pH y potencial de membrana, su sensibilidad a activadores e inhibidores, así como proponer la energía que mueve el proceso de transferencia a través de la membrana plasmática.
- El incorrecto funcionamiento de los sistemas de transporte hepatobiliar de ácidos biliares es responsable de colestasis hepatocelular. En este objetivo se abordó la investigación del efecto de metabolitos sulfatados de progesterona, que se encuentran elevados durante la gestación normal y aún más en determinadas situaciones subclínicas (hipercolanemia asintomática de la embarazada) y clínicas (colestasis gestacional), sobre los procesos de captación hepática de ácidos biliares que, junto a la inhibición de los procesos de secreción a nivel canalicular, podrían estar implicados en la etiopatogenia de diferenetes grados de colestasis hepatocelular en estas pacientes.
- Un aumento o una disminución en la actividad transportadora de diversos miembros de las familias de transportadores hepatobiliares expresados en tumores hepáticos puede condicionar el contenido intracelular de fármacos citostáticos y afectar a la respuesta al tratamiento antitumoral. Por ello, en el tercer objetivo de este trabajo nos propusimos desarrollar una herramienta rápida y automatizable para la evaluación del perfil génico del transportoma en tumores hepáticos y digestivos. Dado que la herramienta utilizada TLDAs lo permitía decidimos ampliar el estudio a otras familias de genes implicados en los diferentes mecanismos de quimiorresistencia para poder predecir la falta de respuesta a un determinado tratamiento antitumoral. Este tipo de análisis individualizado puede ser de gran utilidad clínica pues podría evitar la aplicación de terapias estandarizadas que en un determinado paciente no sólo no tendrían efecto antitumoral sino que además podrían inducir una selección de las células más quimiorresistentes y agresivas, lo que limitaría el posible éxito de sucesivas intervenciones farmacológicas. [EN]The overall objective of this thesis is to deepen the study of several issues related to the mechanism of operation of these hepatic transport proteins in order to respond to several questions of interest in areas such as Physiology, Pathophysiology and Pharmacology. This has led to define three sub-goals in the development of this work:
- To investigate the transport mechanism mediated by OATP1B1 and OATP1B3. For this purpose we employed different experimental models have helped to clarify aspects of their dependence on changes in pH and membrane potential, their sensitivity to activators and inhibitors, and propose the energy transfer process moves through the plasma membrane .
- The improper functioning of hepatobiliary transport systems for bile acids is responsible for hepatocellular cholestasis. This objective was approached by the research of the effect of sulfated metabolites of progesterone, which are elevated during normal pregnancy and even more in certain situations subclinical (asymptomatic hipercolanemia pregnant) and clinical (gestational cholestasis), on hepatic uptake processes bile acid, together with the inhibition of processes at the canalicular secretion may be implicated in the pathogenesis of hepatocellular cholestasis diferenetes degrees in these patients.
- An increase or decrease in transporter activity of various members of the families of hepatobiliary transporters expressed in liver tumors can influence the intracellular content of cytostatic drugs and affect the response to antitumor treatment. Therefore, the third objective of this work was aimed at developing a fast, automated tool for evaluating the gene profile of transportoma liver and digestive tumors. Since the tool used permitted TLDAs decided to expand the study to other families of genes involved in different mechanisms of chemoresistance to predict failure to respond to a specific anti-tumor therapy. This type of individual analysis may be useful clinically as it could avoid the application of standard therapies that a patient would not only anti-tumor effect but also could induce a selection of cells more chemoresistant and aggressive, which would limit the possible subsequent success of pharmacological interventions.
