Vértigo subjetivo crónico

Abstract

[ES] Presentamos un extracto del artículo publicado por Staab JP en Continuum Lifelong Learning Neurol (2012; PMID 23042063), sobre el vértigo subjetivo crónico. En 1986, los neurólogos alemanes Thomas Brandt y Marianne Dietrich describen el “síndrome del vértigo postural fóbico” (VPF), a partir de la observación clínica de pacientes con mareo no vertiginoso y exploración neuro-otológica normal. A comienzos del año 2000 un equipo americano dirigido por Jeffrey Staab y Michael Ruckenstein confirmó los síntomas cardinales y su relación con alteraciones del comportamiento, denominándolo “vértigo subjetivo crónico” (VSC). Investigaciones recientes llevadas a cabo en Europa, USA y Japón han validado este síndrome, estando de acuerdo en los síntomas primarios que lo caracterizan, en los factores precipitantes, en su largo curso clínico y en el diagnóstico diferencial; desarrollando prometedoras estrategias de tratamiento con antidepresivos serotoninérgicos, habituación vestibular y en ocasiones psicoterapia complementaria, lo que ha permitido elaborar nuevas hipótesis fisio-patológicas respecto al inicio y mantenimiento de este proceso. En los centros hospitalarios especializados, el VSC se reconoce en los pacientes que acuden con síntomas vestibulares, como el segundo diagnóstico más frecuente, después del vértigo posicional paroxístico benigno (VPPB). [EN] We present a summary of the article published by Staab JP in Continuum Lifelong Learning Neurol (2012; PMID 23042063), on chronic subjective dizziness. In 1986, the German neurologists Thomas Brandt and Marianne Dietrich described the "syndrome of phobic postural vertigo" (PPV), from clinical observation of patients with nonvertiginous dizziness and normal neuro-otologic scan. At the beginning of the year 2000 an American team led by Jeffrey Staab and Michael Ruckenstein confirmed the core physical symptoms and their relationship with changes in behavior, called "chronic subjective dizziness" (CSD). Recent research carried out in Europe, USA and Japan have validated this syndrome, agreeing on the primary symptoms, precipitating factors, its long clinical course and the differential diagnosis; developing successful strategies of treatment with serotonergic antidepressant, vestibular habituation and sometimes complementary psychotherapy, which has allowed new hypotheses about pathophysiologic process that initiate and maintain the disorder. In tertiary neuro-otologic centers, the CSD is recognized, in patients who come with vestibular symptoms, such as the second diagnosis more frequent, after benign paroxysmal positional vertigo (BPPV).
