Caracterización genómica de la leucemia mieloblástica aguda: utilidad clínical del perfil molecular en el diagnóstico, pronóstico y monitorización terapéutica

Resumen
[ES]Las alteraciones genéticas que afectan a los mecanismos normales de crecimiento, proliferación y diferenciación celular.
Los hallazgos citogenéticos y moleculares han servido como marcadores diagnósticos para diversos
subtipos de LMA dentro de la clasificación de la Organización Mundial de la Salud (OMS), así como para
establecer grupos de bajo, intermedio y alto riesgo (European LeukemiaNet), con claras diferencias a nivel
de tasas de remisión completa, riesgo de recaída y supervivencia libre de enfermedad y global.
Sin embargo, a pesar de cumplir los criterios recomendados para la categorización y el tratamiento de la
LMA, los pacientes de un mismo subgrupo pueden tener una evolución clínica diferente. Este es el caso
de la LMA de riesgo favorable, en la que no todos los pacientes presentan realmente un buen pronóstico.
De hecho, hasta un 40% de los pacientes con LMA‐CBF termina recayendo, la mitad de los pacientes con
mutaciones en NPM1 sufren recaída durante los 4 primeros años y las mutaciones bialélicas en CEBPα no
evitan una tasa de recaída del 44%. En cuanto a las LPA, pese a las notables mejorías gracias a su
tratamiento dirigido, aún aparecen casos que presentan resistencia o recaídas y, aunque se han
encontrado mutaciones en los genes implicados en la fusión (PML y RARα) en casos de progresión, el
perfil de aberraciones genéticas asociado a los tres subgrupos de LPA con distinto riesgo (score basado
en cifras de plaquetas y leucocitos) aún no ha sido definido.
Hoy en día, existe un creciente interés en detectar en un único test de secuenciación masiva (NGS) toda
la variedad de alteraciones con valor diagnóstico y pronóstico en las LMA (mutaciones,
traslocaciones/inversiones y variaciones en el número de copias). Sin embargo, son pocos los trabajos
que estudian además de mutaciones génicas, el resto de alteraciones de las LMA, ya que todavía existen
limitaciones técnicas para la detección de reordenamientos, por lo que los métodos convencionales
continúan siendo imprescindibles. Para poder aplicar la técnica de NGS al estudio de estas alteraciones
cromosómicas es necesario mejorar el diseño de los paneles y sobre todo simplificar el análisis
informático de modo que pueda incorporarse a la práctica rutinaria del laboratorio clínico.
Por otra parte, las mutaciones encontradas en las LMA no son solo adquiridas o somáticas, sino que
también pueden presentarse en la línea germinal, dando lugar a casos de LMA familiares. Gracias al uso
creciente de las nuevas tecnologías de NGS, se van descubriendo cada vez más hemopatías malignas
heredables y genes responsables de ello, habiéndose descrito varios síndromes de predisposición
familiar. De hecho, estos hallazgos clínico‐genéticos se recogen ya en la última clasificación de la OMS
(2016), donde se ha incorporado una nueva categoría diagnóstica provisional que engloba los tumores
mieloides hereditarios. No obstante, la investigación sobre LMA familiar a nivel molecular sigue siendo limitada. Aunque se han descrito casos con mutaciones en CEBPα o GATA2 y casos de trastorno
plaquetario familiar con predisposición a LMA con mutaciones en RUNX1, es difícil una caracterización
molecular completa con las técnicas convencionales, así como la monitorización correcta de los familiares
sanos.
Por último, la detección de enfermedad mínima residual (EMR) constituye uno de los factores pronósticos
post‐tratamiento más importantes a la hora de tomar decisiones terapéuticas, ya que la ausencia de EMR
aún no se ha traducido en la predicción inequívoca de curación de la enfermedad. Hay que destacar que
solo el 40‐60% de los pacientes con LMA tienen un marcador de EMR robusto cuantificable por PCR, por
lo que existe un número importante de pacientes en los cuales es imposible la monitorización molecular
de la enfermedad.
En este contexto, nos proponemos confirmar las siguientes hipótesis:
1. La variabilidad en la evolución de la LMA de bajo riesgo genético se debe a las características
biológicas del clon tumoral: presencia de mutaciones o expresión anómala de diferentes
genes. Un estudio genómico y de expresión génica de la LMA de bajo riesgo, permitirá
esclarecer las posibles alteraciones responsables del comportamiento más agresivo de la
enfermedad en dicho subgrupo.
2. El análisis genómico para la detección simultánea de reordenamientos cromosómicos y
mutaciones somáticas de las LMA mediante un único panel de secuenciación masiva,
permitirá demostrar sus ventajas sobre las técnicas citogenéticas y moleculares
convencionales. Su implementación en el laboratorio clínico constituirá una estrategia
innovadora para mejorar el manejo de los pacientes.
3. El empleo de paneles de NGS dirigidos es útil para analizar las mutaciones en genes
relacionados con predisposición genética en los casos de LMA familiar, permitiendo la
identificación de miembros en riesgo o afectados dentro de una familia. Además, se podrá
determinar el origen germinal de la enfermedad y así plantear una estrategia terapéutica
adaptada en la que se elimine la opción de trasplante alogénico emparentado.
4. El estudio de la sobreexpresión de los genes WT1, PRAME y BAALC en la monitorización de
EMR resultará útil en los pacientes en los que no se dispone de un marcador fiable para
seguimiento.
Las conclusiones derivadas de los resultados obtenidos fueron las siguientes:
PRIMER TRABAJO — En relación con la caracterización molecular de las LMA de bajo
riesgo genético (LPA, LMA‐CBF, LMA‐NPM1 y LMA‐biCEBPΑ):
1. El estudio de expresión génica mediante tarjetas microfluídicas es útil para identificar pacientes
con alteraciones citogenéticas/moleculares de bajo riesgo, permitiendo discriminar los distintos
subgrupos con gran precisión.
2. El análisis mediante secuenciación masiva dirigida permitió determinar que las categorías
funcionales de genes y su frecuencia de mutación difieren entre los subgrupos de LMA de bajo
riesgo, lo que refleja procesos leucemogénicos distintos. Cabe destacar la alta incidencia de
mutaciones en los genes implicados en metilación del ADN en las LMA‐NPM1, encontrándose
mutados en el 80% de los pacientes.
3. La presencia de ciertas alteraciones moleculares puede explicar la evolución más agresiva de
algunos pacientes de bajo riesgo, como la sobreexpresión de CD34 y las mutaciones en los genes
de splicing en las LMA‐NPM1, así como las mutaciones en los genes de las cohesinas en las LMACBF.
Sin embargo, el factor pronóstico con mayor valor es el número de mutaciones génicas al
diagnóstico, independientemente de los genes implicados.
4. En nuestra serie, las mutaciones de sobreactivación de genes de la vía RAS al diagnóstico tienen
una influencia negativa en la supervivencia libre de recaída de los pacientes con LMA‐CBF y LPA
de bajo riesgo. De confirmarse estos datos, se podrían plantear nuevos enfoques en el
tratamiento y monitorización terapéutica de estas LMA.
5. El perfil mutacional de las LPA difiere entre los tres grupos con distinto riesgo, clasificados según
las cifras de leucocitos y plaquetas, observándose una mayor incidencia de FLT3‐ITD en el grupo
de alto riesgo.
SEGUNDO TRABAJO — En relación con la nueva estrategia de secuenciación masiva
para la detección de reordenamientos y mutaciones en LMA:
1. La estrategia de NGS basada en captura desarrollada y validada en el presente trabajo permite
analizar en un solo test tanto las mutaciones puntuales como los reordenamientos
(traslocaciones e inversiones) que afectan recurrentemente a las células leucémicas, lo que
supone un avance en el diagnóstico de precisión de estas enfermedades.
2. La metodología de NGS basada en captura se postula como futura técnica de elección para una
caracterización completa de la enfermedad al diagnóstico, ya que además de presentar alta
concordancia con las técnicas convencionales, identifica otras alteraciones no detectadas por las
mismas, como las traslocaciones t(1;17)‐IRF2BP2/RARA y la t (3;21)‐RUNX1/LRRC34, no descritas
hasta la fecha.
3. El algoritmo de libre acceso Lumpy resulta el más adecuado y fiable para la identificación de
traslocaciones e inversiones en datos de NGS. Sin embargo, es necesario implementar
herramientas bioinformáticas de análisis más eficaces y sencillas para el laboratorio clínico de
rutina.
TERCER TRABAJO — En relación con el análisis genético de la LMA familiar:
1. La incorporación de la NGS en la práctica clínica constituye una herramienta muy valiosa para
mejorar el diagnóstico molecular en los casos de LMA familiar.
2. Las alteraciones germinales en el gen RUNX1 son un factor de predisposición a LMA, sin embargo,
no son suficientes para la leucemogénesis, siendo necesarias mutaciones en genes adicionales,
como TP53, para desarrollar el proceso maligno.
3. Existen casos de LMA familiar con mutaciones en RUNX1 sin antecedentes de trombopenia, en los
que el estudio genético resulta de especial interés para asegurar un diagnóstico preciso y evitar
la opción de trasplante alogénico emparentado.
CUARTO TRABAJO — En relación con la sobreexpresión de WT1, PRAME y BAALC
como marcadores de enfermedad mínima residual (EMR) en pacientes con LMA no
promielocítica:
1. La cuantificación de la expresión de WT1, PRAME o BAALC tras el tratamiento de inducción en
pacientes en remisión completa permite identificar pacientes con un riesgo de recaída
significativamente mayor, lo que sugiere la necesidad de una monitorización más estrecha de los
casos positivos.
2. En los pacientes sometidos a trasplante de progenitores hematopoyéticos, el estudio de PRAME
en muestras previas y el de WT1 en muestras posteriores al trasplante (día +100), permite
seleccionar un grupo de pacientes con un riesgo de recaída significativamente más alto.
3. La cuantificación de los niveles de WT1, PRAME y BAALC, además de los marcadores estándar (NPM1
mutado o tránscrito de fusión), permitieron la detección precoz de más del 90% de las recaídas
analizadas, siendo WT1 el marcador con mayor tasa de falsos negativos.
4. La sobreexpresión de WT1, PRAME y BAALC es estable entre el diagnóstico y la recaída y su
evolución es comparable con la de otros marcadores estándar, por lo que pueden emplearse
para la detección de EMR en pacientes que no disponen de un marcador robusto
