Mecanismos de control de costes en el sistema sanitario chileno

Abstract

Este trabajo tiene doble propósito. El primero es analizar los fundamentos teóricos de los instrumentos de contención de costes implementados a lo largo de los últimas dos décadas y su efectividad en el sistema sanitario chileno. Segundo, evaluar cuantitativamente los efectos que han tenido los mecanismos de pago recientemente creados sobre la utilización de los servicios hospitalarios de la Región Metropolitana (RM) y demás regiones del país. Los resultados ponen de manifiesto que el primer mecanismo de pago prospectivo sí ha cumplido con las expectativas de contención en el uso de los recursos, sin embargo el segundo mecanismo de pago no cumplió con dichas expectativas. Como conclusión general podemos decir que el segundo mecanismo de pago introducido, además de no generar incentivos en la reducción de las estancias promedio a nivel nacional tampoco ha generado niveles de eficiencia mas altos. Es decir, no sólo se utilizan más recursos, sino que además es posible que el sistema esté actuando de forma perversa dificultando la adopción de medidas que favorezcan la eficiencia.

This work has got a double purpose. The first one is analyse the theoretical foundations of the instruments of restraint of the implemented cost during the last two decades and their effectiveness on the Chilean sanitary system. The second one, evaluate quantitatively the effect that the mechanisms of payment recently created have got on the use of hospital services in the Metropolitan Region and their regions in the country. The results reverald that the first mechanism of prospective payment has fulfilled the expectations of restraint in the use of the resources however. The second mechanism of payment didn t fulfil these expectations. In conclusion we can say that the second mechanism of payment has generated neither incentives in the reduction of the averaged stay in the country nor higher levels of efficiency. That is to say, not only more resources are used but also it is possible that the system is acting in a wicked way making difficult to take measures that favour the efficiency.
