<dc:description>[ES]El carcinoma de células renales (CCR) es la más frecuente y letal de todos los&#xd;
cánceres genitourinarios, suponiendo el 2-3% de la totalidad de las&#xd;
enfermedades malignas del adulto. Ésta entidad presenta una predilección&#xd;
especial por la afectación vascular, pudiendo extenderse a través de la vena&#xd;
cava inferior (VCI) alcanzando las cámaras cardiacas derechas en el 4-10% y&#xd;
1 % de los casos, respectivamente. La nefrectomía radical (NR) con&#xd;
trombectomía tumoral (TT) se mantiene como la única estrategia terapéutica&#xd;
viable para la obtención de periodos largos libres de enfermedad, con&#xd;
supervivencias que alcanzan el 65% a 5 años en candidatos aceptables. Sin&#xd;
embargo, debido a su complejidad técnica, la incidencia de complicaciones&#xd;
postoperatorias (CPs) tras la NR y TT dista aún de ser óptima, incluyendo en&#xd;
algunos casos la hemorragia masiva (7.5%), el embolismo pulmonar (1.5-&#xd;
3.4%), y la muerte (7.2%).&#xd;
Recientemente, se ha introducido un abordaje quirúrgico basado en el&#xd;
trasplante (BT), con el objetivo de disminuir la incidencia de CPs. El abordaje&#xd;
basado en las maniobras procedentres de la cirugía del trasplante se&#xd;
encuentra definido por una serie de características técnicas que incluyen el&#xd;
empleo de una incisión trirradiada de Chevron, un separador autoestático&#xd;
específico para lograr una adecuada exposición, un conjunto de maniobras&#xd;
vasculares y de movilización visceral, una manipulación específica de la VCI y&#xd;
la trombosis, y la no utilización de acceso al tórax/maniobras de circulación&#xd;
extracorpórea en la medida de lo posible.&#xd;
Dada la escasa prevalencia del CCR con afectación de la VCI, la información&#xd;
sobre CPs publicada se ha limitado mayoritariamente a pequeñas experiencias&#xd;
aisladas de centros únicos, que contienen escasa información desde el punto&#xd;
de vista multivariable, de escasa potencia estadística, y por tanto, poco&#xd;
generalizables. Con el objetivo, entre otros, de proporcionar una evaluación&#xd;
más profunda acerca de los predictores potenciales de CPs tras la realización&#xd;
de una NR con TT, se creó el International Renal Cell Carcinoma-Venous&#xd;
Thrombus Consortium (IRCC-VTC).</dc:description>
<dc:description>[EN]Renal cell carcinoma (RCC) represents the third most frequent and most lethal genitourinary cancer, accounting for 2-3% of all malignant diseases in adults. This malignancy presents a unique proclivity for vascular involvement, extending into the inferior vena cava (IVC) and reaching into the right cardiac chambers in 4-10% and 1% of cases, respectively. Radical nephrectomy (RN) and tumor thrombectomy (TT) remain the only therapeutic strategy able to provide long-term freedom-from-disease, with patient survival as high as 65% at 5 years in acceptable candidates. However, due to its technical complexity, the incidence of postoperative complications (PCs) following RN and TT remains far from optimal, including in some cases massive hemorrhage (7.5%), pulmonary embolism (1.5-3.4%), and death (7.2%). An organ transplant-based (TB) approach in performing TT was recently introduced with the goal of reducing PC incidence. The TB approach is defined by a number of differential operative characteristics, including the use of a triradiate Chevron incision, a specific self-retaining retractor for exposure, a set of visceral mobilizations and vascular maneuvers, IVC and thrombus handling, and avoidance of thoracic access/extracorporeal circulation whenever possible. Given the low prevalence of RCC involving the IVC, PC reporting has been limited to mostly small, single-center experiences containing scarce multivariable information, not readily generalizable, and statistically underpowered. In an attempt to provide a more comprehensive assessment of potential predictors of PCs following RN and TT, the International Renal Cell Carcinoma-Venous Thrombus Consortium (IRCC-VTC) was established.</dc:description>
