<dc:description>En este trabajo se publicó la experiencia de un ensayo multicéntrico fase II, para evaluar la activi-dad de la combinación de bortezomib, dexametasona y rituximab (BDR) en pacientes sintomáticos con macroglobulinemia de Waldenström (WM) que no habían sido tratados previamente. Para prevenir el fenómeno "flare" de la inmunoglobulina M (IgM), el bortezomib se administró como agente único los 21 primeros días (1,3 mg/m2 IV días 1, 4, 8 y 11) seguido de 4 ciclos con la asociación de bortezomib intravenoso semanal (1,6 mg/m2 días 1, 8, 15 y 22) con dexametasona intravenosa (40 mg) cada 35 días durante 4 ciclos adicionales, y rituximab IV semanal (375 mg/m2) en los ciclos 2 y 5. Se trataron  cincuenta y nueve pacientes: el 45,5% y el 40% fueron de riesgo alto e intermedio según el Sistema Internacional de Puntuación Pronóstica para la MT. La respuesta global por intención de tratamiento fue del 85% (3% respuesta completa, 7% respuesta parcial muy buena, 58% respuesta parcial [RP]). En el 11% de los pacientes se observó un au-mento (flare) de IgM ≥25% después de rituximab; Ningún paciente requirió plasmaféresis. Des-pués de un seguimiento mínimo de 32 meses, la mediana de supervivencia libre de progresión fue de 42 meses, la duración de la respuesta a 3 años para los pacientes con ≥RP fue del 70 % y la supervivencia a 3 años fue del 81%. La neuropatía periférica se presentó en el 46% (grado ≥3 en el 7%); Solo el 8% suspendió el bortezomib debido a polineuropatía periérica. Por tanto, BDR es una combinación de acción rápida, bien tolerada y no mielotóxica, que inducie respues-tas duraderas en WM no tratada previamente.</dc:description>
<dc:description>[EN]In this phase 2 multicenter trial, we evaluated the activity of bortezomib, dexamethasone, and rituximab (BDR) combination in previously untreated symptomatic patients with Waldenström macroglobulinemia (WM). To prevent immunoglobulin M (IgM) "flare," single agent bortezomib (1.3 mg/m(2) IV days 1, 4, 8, and 11; 21-day cycle), was followed by weekly IV bortezomib (1.6 mg/m(2) days 1, 8, 15, and 22) every 35 days for 4 additional cycles, followed by IV dexamethasone (40 mg) and IV rituximab (375 mg/m(2)) in cycles 2 and 5. Fifty-nine patients were treated; 45.5% and 40% were high and intermediate risk per the International Prognostic Scoring System for WM. On intent to treat, 85% responded (3% complete response, 7% very good partial response, 58% partial response [PR]). In 11% of patients, an increase of IgM ≥25% was observed after rituximab; no patient required plasmapheresis. After a minimum follow-up of 32 months, median progression-free survival was 42 months, 3-year duration of response for patients with ≥PR was 70%, and 3-year survival was 81%. Peripheral neuropathy occurred in 46% (grade ≥3 in 7%); only 8% discontinued bortezomib due to neuropathy. BDR is rapidly acting, well tolerated, and nonmyelotoxic, inducing durable responses in previously untreated WM.</dc:description>
