[EN] Objective: To evaluate the effectiveness of a primary health care intervention designed to
increase physical activity in people living with dementia and their family caregivers.
Methods: A multicenter cluster-randomized clinical trial was carried out. Participants:
140 people with dementia (median age 82 years; 63.6% women) and 176 caregivers
(median age 62 years; 72.7% women). Seventy people with dementia and 80 caregivers
were assigned to the control group and 70 people with dementia and 96 caregivers to the
intervention group. Physical activity was measured with the pedometer and the
International Physical Activity Questionnaire (IPAQ-SF). The intervention consisted of
applying the previously designed physical activity promotion program (PEPAF) in
primary care for 3 months. The changes observed after 6 months were analyzed.
Results: The changes observed in the physical activity of people with dementia and
caregivers did not reach statistical significance. In the case of people with dementia, the
evaluation through the pedometer revealed a decrease in both groups, although in the
intervention group a significant increase was observed in the number of aerobic steps per
day (52.9 vs. -615.9). The activity reported through the IPAQ-SF questionnaire decreased
more in the intervention group. As for the caregivers, the results of the pedometer
evaluation indicated that both total steps/day (569.4 vs 377.2) and aerobic steps/day
(356.9 vs -12.9) increased more in the intervention group. Regarding the physical activity
questionnaire, a greater increase in physical activity was seen in the intervention group
compared to the control group, with greater consumption of METS per week. When
analyzing physical activity, caregivers in the intervention group performed the same
intense physical activity as the control group. Furthermore, it was found that the number
of evaluations of people with dementia was higher when there was more than one
caregiver involved in the family group, both in the control group (p=0.007) and in the
intervention group (p=0.049). It is important to note that all people with dementia who
had three caregivers participating completed at least two evaluations. Secondarily, no
significant differences were found in changes in functional and cognitive status in people
with dementia or in mental health, Family APGAR and short Zarit in the caregiver group.
Conclusions: Although some positive results were found, in global terms, no differences
were found between the intervention group and the control group in terms of increasing
physical activity. When more dementia caregivers are involved, they are more likely to participate in follow-up, which is relevant for implementation in the primary care setting.
In practice, our results could provide recommendations on the role of mobile technology
(mHealth) in improving physical activity in people with dementia.</dc:description>
