<description>[ES] Objetivo: Evaluar la efectividad de una intervención en atención primaria de salud&#xd;
diseñada para aumentar la actividad física en personas que viven con demencia y sus&#xd;
cuidadores familiares.&#xd;
Métodos: Se llevó a cabo un ensayo clínico multicéntrico aleatorizado por&#xd;
conglomerados.&#xd;
Participantes: 140 personas con demencia (mediana de edad 82 años; 63,6% mujeres) y&#xd;
176 cuidadores (mediana de edad 62 años; 72,7% mujeres). Setenta personas con&#xd;
demencia y 80 cuidadores fueron asignados al grupo de control y 70 personas con&#xd;
demencia y 96 cuidadores al grupo de intervención. La actividad física se midió con el&#xd;
podómetro y con el cuestionario internacional de actividad física (IPAQ-SF). La&#xd;
intervención consistió en aplicar en atención primaria durante 3 meses el programa de&#xd;
fomento de la actividad física (PEPAF) previamente diseñado. Se analizaron los cambios&#xd;
observados a los 6 meses.&#xd;
Resultados: Los cambios observados en la actividad física, en el grupo de intervención&#xd;
respecto al grupo control, tanto en las personas con demencia, como en los cuidadores no&#xd;
alcanzaron la significación estadística. En el caso de las personas con demencia, la&#xd;
evaluación a través del podómetro reveló una disminución de pasos totales en ambos&#xd;
grupos. No obstante, en el grupo intervención se observó un aumento significativo en el&#xd;
número de pasos aeróbicos por día (52,9) mientras que disminuyó en el grupo control (-&#xd;
615,9). La actividad reportada mediante el cuestionario IPAQ-SF disminuyó en ambos&#xd;
grupos, sin diferencia entre ellos. En cuanto a los cuidadores, los resultados de la&#xd;
evaluación con el podómetro indicaron que tanto los pasos totales/día (569,4 vs 377,2)&#xd;
como los pasos aeróbicos/día aumentaron más en el grupo intervención (356,9 vs -12,9),&#xd;
aunque sin alcanzar la significación estadística. En cuanto al cuestionario de actividad&#xd;
física se observó un mayor incremento de actividad física en el grupo de intervención&#xd;
respecto al grupo control, con mayor de METS/minutos a la semana. Al analizar esta&#xd;
variable, la actividad física, los cuidadores del grupo de intervención realizaban la misma&#xd;
actividad física intensa que el grupo control. Además, se encontró que el número de&#xd;
evaluaciones de personas con demencia fue más alto cuando había más de un cuidador&#xd;
involucrado en el grupo familiar, tanto en el grupo control (p=0,007) como en el grupo&#xd;
intervención (p=0,049). Todas las personas con demencia que contaban con tres&#xd;
cuidadores participando, realizaron al menos dos evaluaciones. Secundariamente, no se encontraron diferencias significativas entre ambos grupos en los cambios en el estado&#xd;
funcional y el cognitivo en personas con demencia ni en la salud mental, APGAR Familiar&#xd;
y Zarit breve en el grupo de cuidadores.&#xd;
Conclusiones: Aunque se encontraron algunos resultados positivos, en términos globales,&#xd;
no se encontraron diferencias entre el grupo intervención y el grupo control en cuanto al&#xd;
aumento de la actividad física. Cuando participan más cuidadores por persona con&#xd;
demencia, es más probable que éstos participen en el seguimiento, lo cual es relevante&#xd;
para su implementación en el entorno de atención primaria.&#xd;
[EN] Objective: To evaluate the effectiveness of a primary health care intervention designed to&#xd;
increase physical activity in people living with dementia and their family caregivers.&#xd;
Methods: A multicenter cluster-randomized clinical trial was carried out. Participants:&#xd;
140 people with dementia (median age 82 years; 63.6% women) and 176 caregivers&#xd;
(median age 62 years; 72.7% women). Seventy people with dementia and 80 caregivers&#xd;
were assigned to the control group and 70 people with dementia and 96 caregivers to the&#xd;
intervention group. Physical activity was measured with the pedometer and the&#xd;
International Physical Activity Questionnaire (IPAQ-SF). The intervention consisted of&#xd;
applying the previously designed physical activity promotion program (PEPAF) in&#xd;
primary care for 3 months. The changes observed after 6 months were analyzed.&#xd;
Results: The changes observed in the physical activity of people with dementia and&#xd;
caregivers did not reach statistical significance. In the case of people with dementia, the&#xd;
evaluation through the pedometer revealed a decrease in both groups, although in the&#xd;
intervention group a significant increase was observed in the number of aerobic steps per&#xd;
day (52.9 vs. -615.9). The activity reported through the IPAQ-SF questionnaire decreased&#xd;
more in the intervention group. As for the caregivers, the results of the pedometer&#xd;
evaluation indicated that both total steps/day (569.4 vs 377.2) and aerobic steps/day&#xd;
(356.9 vs -12.9) increased more in the intervention group. Regarding the physical activity&#xd;
questionnaire, a greater increase in physical activity was seen in the intervention group&#xd;
compared to the control group, with greater consumption of METS per week. When&#xd;
analyzing physical activity, caregivers in the intervention group performed the same&#xd;
intense physical activity as the control group. Furthermore, it was found that the number&#xd;
of evaluations of people with dementia was higher when there was more than one&#xd;
caregiver involved in the family group, both in the control group (p=0.007) and in the&#xd;
intervention group (p=0.049). It is important to note that all people with dementia who&#xd;
had three caregivers participating completed at least two evaluations. Secondarily, no&#xd;
significant differences were found in changes in functional and cognitive status in people&#xd;
with dementia or in mental health, Family APGAR and short Zarit in the caregiver group.&#xd;
Conclusions: Although some positive results were found, in global terms, no differences&#xd;
were found between the intervention group and the control group in terms of increasing&#xd;
physical activity. When more dementia caregivers are involved, they are more likely to participate in follow-up, which is relevant for implementation in the primary care setting.&#xd;
In practice, our results could provide recommendations on the role of mobile technology&#xd;
(mHealth) in improving physical activity in people with dementia.</description>
