<title>Impacto de la Farmacogenética en la Medicina Personalizada de Precisión aplicada a la Prescripción Terapéutica</title>

<description>[ES] La seguridad del paciente en la prescripción farmacológica es un objetivo&#xd;
primordial en la asistencia sanitaria y está directamente relacionada con la respuesta&#xd;
individual al tratamiento. Dicha respuesta está condicionada en gran medida por los&#xd;
factores genéticos. En este sentido, la Farmacogenética se basa en el estudio de las&#xd;
variantes génicas que influyen en la respuesta individual a los fármacos. Surge así la&#xd;
Medicina Personalizada de Precisión, como un nuevo enfoque respecto a la Medicina&#xd;
Clásica, centrado en el paciente de forma individual. En este trabajo nos proponemos&#xd;
estudiar las características específicas de los pacientes a los que se les ha realizado el&#xd;
estudio farmacogenético para el ajuste racionalizado de la medicación. Para ello nos&#xd;
planteamos analizar las características clínico-biológicas, la prescripción farmacológica&#xd;
y el perfil farmacogenético de los pacientes para finalmente evaluar la relevancia clínica&#xd;
de los resultados obtenidos. Así mismo, hemos realizado una aproximación del estudio&#xd;
de variantes estructurales de CYP2D6 y una comparación de los resultados obtenidos por&#xd;
medio de dos estrategias de genotipado. Además, la información disponible en la&#xd;
actualidad sobre recomendaciones internacionales se emite en relación con los fármacos&#xd;
individuales, y, sin embargo, los pacientes se presentan en su mayoría en un contexto de&#xd;
politerapia. Por todo ello, el estudio se ha realizado mediante la aplicación de un modelo&#xd;
propio denominado 5SPM de Medicina de Precisión, en 5 pasos, utilizando el genotipado&#xd;
de marcadores mediante técnicas de Biología Molecular y el posterior análisis in silico&#xd;
de los datos. Un modelo internacionalmente innovador que enfoca el ajuste terapéutico&#xd;
considerando el contexto de la polimedicación.&#xd;
En este trabajo hemos obtenido varios resultados con relevancia clínica en&#xd;
relación con distintos marcadores como CYP2C9, CYP3A5 y CYP2D6 y hemos&#xd;
identificado la presencia de un porcentaje importante de pacientes portadores de&#xd;
genotipos asociados a disminución de la actividad enzimática de alguno de los&#xd;
marcadores estudiados, lo que pone de manifiesto la enorme importancia de realizar&#xd;
estudios farmacogenéticos actualizados, en especial en aquellos pacientes polimedicados.&#xd;
[EN] One of the main objectives in health care is patient safety in pharmacological&#xd;
prescription, which directly correlates to the individual treatment response. Genetic&#xd;
factors largely condition this response. In this way, Pharmacogenetics is based on the&#xd;
study of genetic variants that influence the individual response to drugs. Thus, arises&#xd;
Personalized Precision Medicine as a new approach to Classical Medicine, focused on&#xd;
the patient’s individuality. In this study we will try to study the specific characteristics of&#xd;
the patients treated using a pharmacogenetic study for the rational adjustment of the&#xd;
medication. In order to do so, we propose to analyze the biological-clinical&#xd;
characteristics, the pharmacological prescription and the pharmacogenetic profile of the&#xd;
patients, finally evaluating the clinical relevance of the results that have been obtained.&#xd;
Likewise, we have carried out an approach to studying structural variations of CYP2D6&#xd;
and comparing the results obtained through different types of genotyping. In addition, the&#xd;
available international information in mainly related to individual drugs, however, in&#xd;
actual life, patients are under polypharmacy. Considering that, this study carried out&#xd;
applying 5SPM of Precision Medicine in 5 steps, an own model, using the&#xd;
pharmacogenotyping techniques and the subsequent in silico data analysis. This is an&#xd;
internatonial innovative model owing to the fact that it takes into account the complete&#xd;
context of patient therapy. In this work, we have obtained results with clinical relevance&#xd;
related to different markers such as CYP2C9, CYP3A5, and CYP2D6, and we have&#xd;
identified a significant amount of patients that have genotypes associated with a decrease&#xd;
in the enzymatic activity of some of the studied markers, which shows the enormous&#xd;
importance of doing innovative pharmacogenetic studies, especially in polymedicated&#xd;
patients.</description>
