<mods:abstract>[ES]Introducción:&#xd;
Los politraumatismos taurinos forman parte de la práctica clínica del CAU de Salamanca.&#xd;
Las lesiones provocadas por un toro suelen ser consideradas una urgencia médica, al&#xd;
tratarse habitualmente de pacientes politraumatizados de alta energía, que requieren con&#xd;
frecuencia asistencia quirúrgica y cuidados intensivos. Por tanto, es esencial conocer el&#xd;
manejo inicial del politrauma y del sangrado para disminuir la morbimortalidad de este&#xd;
tipo de traumatismo.&#xd;
Justificación:&#xd;
Las publicaciones sobre los traumatismos relacionados con el mundo taurino no son muy&#xd;
abundantes en la literatura médica, pero estos representan un problema de interés en&#xd;
ciudades con tradición taurina como Salamanca.&#xd;
Objetivos:&#xd;
Describir los traumatismos relacionados con el mundo taurino, su epidemiología, los&#xd;
factores de riesgo y su manejo, así como las características, evolución y situación al alta&#xd;
de pacientes ingresados en la UCI del CAUSA.&#xd;
Material y métodos:&#xd;
Revisión de la literatura sobre el traumatismo relacionado con el mundo taurino basada&#xd;
en diversas fuentes como PubMed y ScienceDirect de Elsevier.&#xd;
Estudio observacional retrospectivo de pacientes ingresados por traumatismo taurino en&#xd;
la UCI del CAUSA, realizando una comparativa entre dos cohortes según el tipo de&#xd;
traumatismo, ya sea contuso o penetrante.&#xd;
Los pacientes que se incluyen en el estudio son 30 pacientes extraídos de la base de datos&#xd;
del RETRAUCI desde enero de 2015 a julio de 2024.&#xd;
Las variables de interés que se analizan son: demografía, mecanismo lesional, atención&#xd;
prehospitalaria y hospitalaria, estado basal del paciente , descripción de lesiones y estado&#xd;
de gravedad mediante diferentes escalas, constantes y otros parámetros de&#xd;
monitorización, recursos utilizados (cirugías, transfusiones, ventilación, pruebas&#xd;
complementarias…), complicaciones, posibilidad de supervivencia, evolución, tiempo de&#xd;
estancia en la UCI, resultados clínicos y situación al alta.&#xd;
Resultados:&#xd;
De los 30 pacientes, el 93,3% son varones y la media de edad es de 51,27 ±19,17 años.&#xd;
Predomina el trauma contuso (60%) frente al penetrante (40%).&#xd;
La mayoría de contusos fue de origen laboral (61,1%), a diferencia de los penetrantes&#xd;
(25%). El 6,7% presentó alcohol en sangre.&#xd;
La media de los pacientes en la ISS es de 15,33±5,9, y de 19,97±7,4 en la NISS, siendo&#xd;
la media de ambas escalas superior en los traumas contusos respecto a los penetrantes.&#xd;
La posibilidad de supervivencia es de 90,87%±15,89, siendo menor en el trauma contuso.&#xd;
Según la clasificación MAIS, las áreas corporales más afectadas en los traumas contusos&#xd;
son el tórax y abdomen, en cambio, en los penetrantes, son las extremidades.&#xd;
El 13,3% necesitó transfusión de concentrado de hematíes.&#xd;
La ventilación mecánica la necesitó el 44,4% de los traumas contusos y el 58,3% de los&#xd;
penetrantes. Los 2 pacientes que más tiempo requirieron ventilación mecánica (ambos&#xd;
contusos), también requirieron monitorización de la PIC y craniectomía descompresiva.&#xd;
De los traumas contusos, el 33,3% necesitó cirugía urgente a diferencia de los penetrantes&#xd;
(91,7%), siendo la abdominal la más frecuente en ambos.&#xd;
Las complicaciones más frecuentes fueron: alteración hemodinámica (40%),&#xd;
coagulopatía postraumática (26,7%), disfunción respiratoria (26,7%), rabdomiólisis&#xd;
(23,3%), SDMO (13,3%), hemorragia masiva (10%), insuficiencia renal (10%), HIC&#xd;
(6,7%) e infección de tipo nosocomial (6,7%).&#xd;
La media de estancia en la UCI fue de 5,07±6,46 días.&#xd;
El 93,3% tuvo buena evolución, recibiendo el alta a la planta y, sólo hubo 2 éxitus, en los&#xd;
que se limitó el esfuerzo terapéutico.&#xd;
Conclusión:&#xd;
Los traumatismos por asta de toro se consideran traumatismos de alta energía y de&#xd;
urgencia médica al ser politraumatizados. Suelen ser pacientes que sufren un gran número&#xd;
de complicaciones que suponen un consumo elevado de recursos, por lo que es esencial&#xd;
conocer la secuencia de X-ABCDE de los protocolos de ATLS y, el conocimiento del&#xd;
manejo y control de la hemorragia según la guía europea para disminuir la&#xd;
morbimortalidad.</mods:abstract>

Bullfighting polytraumatisms are part of the clinical practice of the Salamanca CAU.&#xd;
Injuries caused by a bull are usually considered a medical emergency as they are highenergy&#xd;
polytraumatized patients who frequently require surgical assistance and intensive&#xd;
care. Therefore, it is essential to know the initial management of polytrauma and bleeding&#xd;
to reduce the morbidity and mortality of this type of trauma.&#xd;
Justification:&#xd;
Publications on bullfighting-related traumatisms are not very abundant in the medical&#xd;
literature, but these represent a problem of interest in cities with a bullfighting tradition&#xd;
such as Salamanca.&#xd;
Objectives:&#xd;
To describe bullfighting-related trauma, its epidemiology, risk factors and management,&#xd;
as well as the characteristics, evolution and situation at discharge of patients admitted to&#xd;
the ICU of the CAUSA.&#xd;
Material and methods:&#xd;
Literature review on trauma related to the bullfighting world based on various sources&#xd;
such as PubMed and Elsevier's ScienceDirect.&#xd;
Retrospective observational study of patients admitted for bullfighting trauma in the ICU&#xd;
of the CAUSA, making a comparison between two cohorts according to the type of&#xd;
trauma, either blunt or penetrating.&#xd;
The patients included in the study are 30 patients extracted from the RETRAUCI database&#xd;
from January 2015 to July 2024.&#xd;
The variables of interest analyzed are: demographics, mechanism of injury, pre-hospital&#xd;
and hospital care, baseline patient condition, description of injuries and severity using&#xd;
different scales, constants and other monitoring parameters, used resources (surgery,&#xd;
transfusions, ventilation, complementary tests...), complications, survival rate, evolution,&#xd;
length of stay in the ICU, clinical outcomes and discharge status.&#xd;
Results:&#xd;
Of the 30 patients, 93.3% were male and the mean age was 51.27±19.17 years.&#xd;
Blunt trauma was predominant (60%) as opposed to penetrating trauma (40%).&#xd;
Most blunt injuries were of occupational origin (61.1%), in contrast to penetrating injuries&#xd;
(25%). Blood alcohol was present in 6.7%.&#xd;
The mean of the patients on the ISS is 15.33±5.9, and 19.97±7.4 on the NISS, with the&#xd;
mean of both scales being higher for blunt trauma than for penetrating trauma.&#xd;
The survival rate is 90.87%±15.89 and it is lower in blunt trauma.&#xd;
According to the MAIS classification, the most affected body areas in blunt trauma are&#xd;
the thorax and abdomen, while in penetrating trauma, it is the extremities.&#xd;
13.3% required transfusion of red blood cells concentrate.&#xd;
Mechanical ventilation was required in 44.4% of blunt traumas and in 58.3% of&#xd;
penetrating traumas. The 2 patients who required mechanical ventilation for the longest&#xd;
time (both-contuse) also required ICP monitoring and decompressive craniectomy. Of the&#xd;
blunt traumas, 33.3% required urgent surgery as opposed to the penetrating ones (91.7%),&#xd;
with abdominal surgery being the most frequent in both.&#xd;
The most frequent complications were haemodynamic alteration (40%), postraumatic&#xd;
coagulopathy (26.7%), respiratory dysfunction (26.7%), rhabdomyolysis (23.3%),&#xd;
SDMO (13.3%), massive haemorrhage (10%), kidney failure (10%), ICH (6,7%) and&#xd;
nosocomial infection (6.7%).&#xd;
The mean length of stay in the ICU was 5.07÷6.46 days.&#xd;
The 93.3% of patients had a good evolution, being discharged to the ward and, there were&#xd;
only 2 exitus, in which the therapeutic effort was limited.&#xd;
Conclusion:&#xd;
Bull horn injuries are considered high-energy trauma and medical emergencies due to the&#xd;
fact that they are polytraumatized. They are usually patients who suffer a large number of&#xd;
complications that involve a high consumption of resources, so it is essential to know the&#xd;
sequence of X-ABCDE of the ATLS protocols and, the knowledge of the management&#xd;
and control of bleeding according to the European guideline to reduce morbidity and&#xd;
mortality.</mods:abstract>
