Resumen
La enfermedad de Gaucher (GD) es el trastorno de almacenamiento lisosomal más común en todo el mundo y fue descrita por primera vez por Philippe Gaucher en 1882.
El sustrato glucosilceramida se acumula dentro de los macrófagos tisulares también conocidos como células de Gaucher, y se pueden encontrar principalmente en el bazo y el hígado, seguido de médula ósea, osteoclastos, piel, pulmones... Esta enfermedad de almacenamiento se debe a una deficiencia del ácido enzimático lisosómico β-glucosidasa, que en condiciones normales puede hidrolizar glucosilceramida a glucosa y ceramida.
La razón por la cual hay una deficiencia en la actividad de la β-glucosidasa ácida es una mutación en el gen GBA1, que codifica la enzima anterior. Hay más de 200 mutaciones diferentes para GBA1, pero solo algunas de ellas son las más comunes: N370S, L444P, 84GG y D409H. Como es un trastorno autosómico recesivo, la consanguinidad puede aumentar el riesgo de padecer la enfermedad de Gaucher, como se puede ver en los judíos asquenazíes.
Existen tres variedades diferentes de la enfermedad de Gaucher dependiendo de la mutación y si hay homocigosidad o heterocigosidad: forma visceral (enfermedad de Gaucher tipo 1), forma neuropática aguda (enfermedad de Gaucher tipo 2) y forma neuropática crónica (enfermedad de Gaucher tipo 3). Por lo tanto, existe una fuerte correlación entre el genotipo de la mutación y el fenotipo que se expresará en el futuro.
Aunque hace algunas décadas el único tratamiento era de alivio sintomático (esplenectomía, analgésicos ...), hoy en día existe un nuevo tipo de tratamiento que ha aumentado la esperanza de vida y la calidad de vida en pacientes con Gaucher: terapia de reemplazo enzimático. Consiste en introducir una β-glucosidasa ácida modificada del ovario del hámster chino a través de una infusión intravenosa. Además, hay otro tratamiento llamado terapia de reducción de sustrato, pero solo se usa en caso de que el primero no funcione correctamente. Ambos tratamientos solo se recomiendan para aquellos pacientes con diabetes tipo 1 o tipo 3. Todavía no hay un tratamiento disponible para pacientes con diabetes tipo 2, ya que la muerte ocurre antes de los dos años.
