Resumen
[ES] El trastorno del espectro autista (TEA) es una condición que afecta a toda la vida
de la persona, y se caracteriza por déficits significativos en la comunicación
socioemocional y por comportamientos repetitivos y restringidos o intereses
específicos. Así como, por una funcionalidad afectada en diferentes áreas de la vida
cotidiana, las cuáles son más limitantes al alcanzar la adultez. Desconocemos con
precisión qué factores están detrás de este funcionamiento limitado. No obstante, esta
falta de conocimiento tiene importantes implicaciones para la conceptualización del
TEA, especialmente para comprender su heterogeneidad, analizar la eficacia de las
intervenciones terapéuticas y desarrollar instrumentos que sirvan para el diagnóstico.
Para contribuir a reducir este vacío en la investigación en el ámbito de la vida
adulta en el TEA, en la presente tesis doctoral consideramos clave acoger un enfoque
dimensional para analizar y comprender la heterogeneidad que se observa en estos
individuos. Consideramos que detrás de las diferentes presentaciones clínicas de este
trastorno y de su funcionamiento limitado se encuentra una serie de dimensiones, que
de una u otra manera, contribuyen positiva o negativamente a las características del
TEA. Como, por ejemplo, el nivel de lenguaje, el grupo de edad en el que se encuentra
el individuo, su inteligencia y por último creemos que las funciones ejecutivas (FE)
juegan un papel muy importante en relación con las habilidades y funcionamiento
adaptativo. Es importante profundizar en el alcance que tienen las FEs en la capacidad
de realizar tareas cotidianas que le permitan a la persona con TEA adaptarse al ambiente
y valerse por sí mismos. Para evaluar esta hipótesis, argumentamos que otras
condiciones co-ocurrentes aumentan la heterogeneidad del TEA, afectando los resultados funcionales de los adultos con TEA. Razón por la cual incorporamos a este
trabajo de investigación no solo a un grupo control de individuos con un desarrollo
típico, sino que también incluimos a un grupo de individuos que tienen una presentación
clínica similar a la que se observa en los adultos con TEA, incluyendo así a individuos con
trastornos del espectro de la esquizofrenia (TEE). Analizar en detalle las dimensiones
antes mencionadas entre ambas condiciones co-ocurrentes nos llevará a desvelar e
identificar cuáles de ellas pueden asociarse a las características sintomáticas del TEA y a
las alteraciones a nivel funcional. Todo esto en su totalidad, abre un camino a los
profesionales de servicios de salud hacia una mejor caracterización al momento de
diagnóstico, así como a diseñar mejores estrategias diagnósticas y de intervención para
conseguir objetivos característicos de la etapa adulta (como, por ejemplo, conseguir un
empleo y vivir de forma autónoma) y conseguir que sean individuos competentes.
Los resultados de esta tesis apuntan a que las FEs tienen un papel predominante
por su implicación con las habilidades adaptativas, las cuales son vitales para conseguir
un funcionamiento apropiado. En los diferentes estudios realizados observamos una
relación significativa ntre estas dos variables e incluso identificamos el valor predictivo
que tienen las FEs sobre un bajo funcionamiento adaptativo. Patrón que únicamente se
observó en el grupo TEA. Asimismo, trazamos un perfil neuropsicológico de las FEs en
ambos grupos, estableciendo diferencias y similitudes entre ambos grupos, también
identificamos habilidades de la FE que están preservadas. Específicamente, hemos
identificado que el grupo TEA presenta déficits importantes con relación a la FE de
flexibilidad, dificultades para realizar tareas que requieran iniciar, planificar u organizar
tareas o actividades, así como necesitan más tiempo para realizar tareas que requieran
eliminar distractores e inhibir ciertas respuestas no deseadas por mencionar algunas.
Estos hallazgos no solo permiten delinear diferencias en cuanto al proceso diagnóstico,
sino que también dan lugar a diseñar mejores estrategias o programas de intervención
terapéuticas.
