Revisión sobre prevención y tratamiento de la mucositis oral en cáncer de cabeza y cuello

Abstract

Introduction: One of the most serious side effects of radiotherapy and chemotherapy in head and neck cancer is oral mucositis or inflammation of the mucosa of the gastrointestinal tract, causing pain and impossibility of a correct intake. Objective: To investigate the interventions that can be developed by the nursing professional to prevent and treat oral mucositis in patients treated for head and neck cancer. Method: A literature search was carried out in PubMed, CUIDEN, CINAHL, Trip, Cochrane, Medes and Epistemonikos databases. Finally, eight articles were selected in which their qualitative analysis was carried out. Results: The most relevant interventions are good oral hygiene and care, saline rinses, sodium bicarbonate, thyme honey and morphine or doxepin solutions, as well as cryotherapy, phototherapy, gabapentin as an adjuvant and Episil® rinse . The use of chlorhexidine is discouraged. Conclusions: Hygiene and oral care are important for the prevention and treatment of mucositis, helping to carry out radiotherapy or chemotherapy without interruptions. The use of mouthwashes, gabapentin, phototherapy and cryotherapy reduce pain and reduce the severity of mucositis.

Introducción: Uno de los efectos secundarios más graves del tratamiento radioterápico y quimioterápico en el cáncer de cabeza y cuello es la mucositis oral o inflamación de la mucosa del tracto gastrointestinal causando dolor e imposibilidad de una correcta ingesta./nObjetivo: Investigar las intervenciones que puede desarrollar el profesional de enfermería para prevenir y tratar la mucositis oral en pacientes tratados por cáncer de cabeza y cuello.

/nMétodo: Se realizó una búsqueda bibliográfica en las bases de datos PubMed, CUIDEN, CINAHL, Trip, Cochrane, Medes y Epistemonikos. Finalmente se seleccionaron ocho artículos en los que se realizó su análisis cualitativo.

/nResultados: Las intervenciones más relevantes son un buen cuidado e higiene oral, enjuagues salinos, con bicarbonato sódico, con miel de tomillo y con soluciones de morfina o doxepina, así como la crioterapia, la fototerapia, la gabapentina como coadyuvante y el enjuague Episil®. Se desaconseja el uso de clorhexidina.

/nConclusiones: La higiene y cuidado oral son importantes para la prevención y tratamiento de la mucositis ayudando a llevar a cabo el tratamiento radioterápico o quimioterápico sin interrupciones. El uso de enjuagues, la gabapentina, la fototerapia y crioterapia reducen el dolor y disminuyen de la gravedad de la mucositis.
