[ES]Introducción: La hipófisis es una glándula endocrina situada en la base del cráneo, en la
silla turca. Los Tumores Neuroendocrinos Hipofisarios (TNEH) son expansiones clonales
de células adenohipofisarias que pueden originar una enorme heterogeneidad de cuadros
clínicos producidos por el efecto de la masa tumoral y por la producción excesiva de
hormonas. El diagnóstico y tratamiento de estos tumores deben ser abordados por
equipos multidisciplinares de expertos en patología hipofisaria que trabajen en hospitales
de referencia para una población suficientemente numerosa. Estos serían los Centros de
Excelencia en Tumores Hipofisarios (CETH). La cirugía hipofisaria es el tratamiento de
elección en la mayoría de los casos, si bien su objetivo debe individualizarse en cada
paciente dependiendo del tamaño, extensión y funcionalidad del tumor.
Objetivo: Evaluar la eficacia de la cirugía hipofisaria en los 76 pacientes con TNEH
operados en el Complejo Asistencial Universitario de Salamanca (CAUSA) entre 2018 y
2022. Se entiende como eficacia el cumplimiento del objetivo marcado antes de la cirugía,
que es, para los tumores secretores, la curación bioquímica y la resección tumoral y para
los no secretores, la descompresión o la resección macroscópica completa.
Por último, se comparan estos resultados con los de un centro de referencia internacional,
el Hospital Universitario de Tubinga (Alemania).
Material y métodos: Estudio retrospectivo unicéntrico descriptivo de 76 pacientes
intervenidos de TNEH en el CAUSA entre 2018 y 2022. Se realiza una base de datos que
incluye distintas variables clínicas (edad, sexo...) y para cada tipo de tumor (no
productores y productores de GH, ACTH, TSH y/o PRL) se analizaron el tamaño tumoral,
la funcionalidad, la anatomía patológica, el tipo de abordaje, el neurocirujano responsable
y los días de estancia media.
Previamente a la intervención se marcaron los objetivos para cada paciente según la
imagen de la RM hipofisaria y los niveles hormonales en sangre. Tras la cirugía se
constató el logro de esos objetivos mediante una nueva RM hipofisaria, nuevas
determinaciones hormonales y la evaluación clínica.
Resultados: De los 76 pacientes, 55 consiguieron el objetivo que se marcó antes de la
cirugía (72,37%) y en los otros 21 pacientes no se consiguió el objetivo marcado
(27,63%).
Estos pacientes están divididos en 6 grupos: en los tumores no productores se alcanzó el
objetivo en 35 de los 47 casos (74,4%); en los tumores productores de ACTH, en 5 de los
7 casos (71,4 %); en los tumores productores de GH, en 10 de los 14 casos (71,43 %); en
los tumores productores de PRL, en 3 de los 6 casos (50 %); en el único paciente con
tumor productor de TSH se consiguió el objetivo (100%) y lo mismo ocurrió en el
paciente con quiste de la bolsa de Rathke (100%).
Conclusión: Nuestro resultados son concordantes con los hallazgos del mayor estudio
realizado a nivel internacional, a pesar de que la experiencia de los cirujanos del CAUSA
no llega al estándar mínimo exigido en términos de número mínimo de operaciones de
TNEH cada año (16 intervenciones anuales frente a 25). [EN]Introduction: The pituitary is an endocrine gland located at the base of the skull, in the turkish saddler. Pituitary Neuroendocrine Tumors (PNET) are clonal expansions of adenohypophyseal cells that can originate diverse symptoms due to the effect of the tumor mass and / or the excessive production of hormones. The diagnosis and treatment of these tumors should be approached by multidisciplinary teams of experts in pituitary pathology, working in reference hospitals for a sufficiently large population. These teams would be the Centers of Excellence in Pituitary Tumors (CETH). Pituitary surgery is the treatment of choice in most cases, although its objective must be individualized in each patient depending on the size, extension and functionality of the tumor.
Objective: To evaluate the efficacy of pituitary surgery in the 76 patients with PNET operated on at the Complejo Asistencial universitario de Salamanca (CAUSA) between 2018 and 2022. Efficacy is understood as the accomplishment of the surgical goal, which may vary depending on the tumor classification. For secretory tumors the goal is biochemical cure and tumor resection and for non-secretory tumors it would be decompression or complete macroscopic resection. Finally, these results are compared with those of an international reference center, the University Hospital of Tübingen (Germany).
Material and methods: retrospective single-center descriptive study of 76 patients operated on for HSCT at CAUSA between 2018 and 2022. A database including different clinical variables (age, sex...) and for each tumor type (GH, ACTH, TSH, PRL producing and non GH producing) tumor size, functionality, pathological anatomy, type of approach, neurosurgeon responsible and mean days of stay were analyzed.
Prior to the intervention, the objectives were set for each patient according to the image of the pituitary MRI and the hormone levels in blood. After surgery, the achievement of these goals was confirmed by doing a new pituitary MRI, new hormone determinations, clinical hormone determinations and clinical evaluation. Results: Out of the 76 patients studied, 55 achieved the target goal before surgery (72.37%) and the other 21 patients did not achieve the target goal (27.63%).
These patients are divided into 6 groups: in non-producing tumors the target was achieved in 35 out of 47 cases (74.4%); in ACTH producing tumors, in 5 out of 7 cases (71.4 %); in GH producing tumors, in 10 out of 14 cases (66.6 %); in PRL producing tumors, in 3 out of 6 cases (50 %); in the only patient with TSH producing tumor the target was achieved (100%) and the same happened in the patient with Rathke's pouch cyst (100%).
Conclusion: Our results are concordant with the findings of the largest study performed at international level, despite the fact that the experience of CAUSA surgeons does not reach the minimum standard required (in terms of minimum number of HSCT operations per year, 16 annual operations vs. 25).
